Youth Verification of a Clear Criminal Record

This form is designated for parents or legal guardians of applicants age 17 or younger who are seeking to volunteer in the joined ministries of Mountain Creek Community, Calgary True Light Gospel  and Calgary Glory Gospel Churches. It serves as an official document to confirm the identity and eligibility of the youth applicant. By completing and signing this form, the parent or guardian verifies that the youth applicant has no criminal convictions related to violence, sexual offenses, illicit drug use, or property crimes to the best of their knowledge. This form ensures transparency and safety in the selection process for youth applicants within our organization.

By signing below, I confirm that the individual identified above is a youth applicant, aged 17 or younger, who will serve as a volunteer representing our organization. I affirm that, to the best of my knowledge, the aforementioned youth has no criminal record, whether felony or misdemeanor, pertaining to (check each one that applies):
